Before your visit

New patient forms

Save time at your appointment by completing your new patient forms in advance. Download and complete the forms below, then bring the completed copies with you to your appointment.

For your privacy and security, do not email completed forms containing personal or medical information. Please bring them to the clinic unless our staff provides different instructions.

Disability and FMLA Paperwork

Disability and FMLA paperwork is completed as a courtesy to our patients. Please allow up to seven business days for forms to be completed. To ensure accuracy, forms are typically completed as close to the scheduled surgery date as possible.

Forms may be emailed, faxed, or hand-delivered to our Jackson office.

Submit all paperwork as early as possible. However, completed forms may not be available until closer to the scheduled surgery date so that the medical information is current and accurate.

    A photo ID and your current insurance card let us check you in without delay.

  • Medication list with prescription bottles and vitamins

    List your medications

    Write down every prescription, over-the-counter medicine, vitamin, and supplement you take, with doses.

  • Organized medical records and imaging documents

    Gather related records

    Imaging, lab work, biopsy results, and notes from the provider who referred you all help your surgeon.

    Write down your questions

    It is easy to forget a question in the moment. A short list makes your visit more useful.

    Bring someone if you would like

    A family member or friend can help you remember what was discussed and what comes next.

    That leaves time for paperwork, insurance verification, and any questions at the front desk.

Before a procedure

Procedure preparation

Every operation has its own instructions. The guidance below is general; the instructions your surgeon and our staff give you always come first.
  • Follow the eating, drinking, and medication instructions you are given exactly. Some medications, including blood thinners, may need to be adjusted or stopped before surgery.
  • Tell our office about every medication and supplement you take, and about allergies or previous reactions to anesthesia.
  • Arrange a ride home and, when recommended, someone to stay with you for the first day.
  • Plan time away from work or school based on what your surgeon tells you about recovery.
  • Call our office if you develop a fever, a cold, or any new symptom in the days before your procedure.

Emergency symptoms

If you have severe or sudden pain, a high fever, heavy bleeding, chest pain, or trouble breathing, call 911 or go to the nearest emergency room. Do not wait for an office appointment or use a website form.

Coverage and cost

Insurance and billing

Coverage rules differ from plan to plan, including whether a referral or prior authorization is required. Before your visit, it helps to call the number on your insurance card and ask what your plan requires for a surgical consultation.

Accepted insurance plans

A published list of the insurance plans this practice participates in is not available yet. We will post it here once the practice confirms the current list. In the meantime, our business office can answer plan questions by phone.

Frequently asked questions

Do I need a referral to be seen?
Many of our patients are referred by their primary care provider or another specialist, and some contact us directly. Call our office and our staff will tell you what your insurance plan requires and what records we need.
How do I get my new patient paperwork?
Our staff provides your new patient forms when your appointment is scheduled, and forms are also available at the front desk when you arrive. Arriving about fifteen minutes early gives you time to complete them without feeling rushed.
What should I bring to my first visit?
Bring a photo ID, your insurance card, a list of your current medications and doses, and any imaging, biopsy results, or records related to the reason for your visit. If someone is helping with your care, they are welcome to come with you.
Will I need surgery after my consultation?
Not necessarily. A consultation is an evaluation. Your surgeon may recommend surgery, further testing, watchful waiting, or non-surgical management, and will explain the reasoning behind the recommendation.
How do I ask a question about my bill or insurance?
Call our main office and ask for the business office. Have your insurance card and any statement you received with you so staff can look up the details.
How do I request my medical records?
Call our office and ask for medical records. You will be asked to complete a written authorization before records can be released.
